Guidelines

All workout clothing should be modest; shirts and tennis shoes are worn at all times. Jeans can tear the equipment pads and should not be worn.

Madison Body Shop is not responsible for items left unattended.

No outside guest of any age may accompany the member. No one under the age of 15 is allowed. Doors are not to be propped open. You may be asked to show key fob and identify yourself. Do not loan key card/fob to anyone.

When finished, remove weight plates from bars and return all plates, bars, and dumbbells to their correct racks. Please do not drop weight plates, bars, or dumbbells on the floor.

Madison Body Shop reserves the right to add, delete, change or modify guidelines at any time. Madison Body Shop also reserves the right to revoke a membership should the member not comply with the rules.

Membership Conditions

New members are subject to joiner fee. Day passes for a guest accompanied by a member are $5.

Couple rate plan is for 2 people living in the same household.

Family rate plan is 2 adults and youth living in the same household.

All Madison Body Shop members must pay dues on time. Madison Body Shop accepts monthly payment only by automatic ach draft from checking/saving accounts. Members may pay by invoice only in 3, 6 or 12 month increments. If more than 30 days has lapsed between invoiced payments you may be subject to a joining fee.

Membership dues will be collected electronically on the 5th day of each month. Members paying cash/check are due by the 5th of the quarterly, semi or annual due date.

Madison Body Shop memberships are considered continuous until the member notifies management they are discontinuing their membership, or the member does not pay their quarterly, semi or annual rates by due date. Members paying by automatic payment must notify Madison Body Shop 10 days prior to due date of cancellation. All key cards/fobs are Property of Madison Body Shop and must be returned in working order to stop automatic payments.
